Assumption of Risk and Waiver of Liability

In consideration of being allowed to participate in any NXTGENFUTBOL training programs, camps, private sessions, and related activities (collectively, the "Activities"), I, the undersigned parent or legal guardian of the participant named below, acknowledge, understand, and agree to the following:

1. Acknowledgment of Risks

I understand and acknowledge that participation in soccer/futsal training and related athletic activities involves inherent risks, including but not limited to:

• Physical contact with other participants, equipment, and surfaces
• Sprains, strains, fractures, and other musculoskeletal injuries
• Heat-related illness, dehydration, and exhaustion
• Cuts, bruises, and abrasions
• Concussions and other head injuries
• Serious injury, permanent disability, or in rare cases, death

2. Voluntary Participation

I confirm that my child's participation in the Activities is entirely voluntary. I have had the opportunity to ask questions about the Activities and understand the nature of what my child will be participating in.

3. Release and Waiver of Liability

I, on behalf of myself, my child, and our heirs, assigns, and personal representatives, hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE NXTGENFUTBOL, its owners, coaches, staff, volunteers, agents, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in the Activities, or while on the premises where the Activities are conducted.

4. Indemnification

I agree to INDEMNIFY AND HOLD HARMLESS the Released Parties from any loss, liability, damage, or costs, including court costs and attorney fees, that may be incurred due to my child's participation in the Activities, whether caused by the negligence of the Released Parties or otherwise.

5. Medical Authorization

In the event of an emergency, I authorize NXTGENFUTBOL staff to seek and consent to emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for all costs associated with such medical treatment.

6. Parental Responsibility

I understand and agree that:

• Parents/guardians are responsible for their players at all times
• Players must be picked up promptly at the end of each training session
• Parents should inform coaches of any medical conditions, allergies, or special needs
• Appropriate athletic attire and equipment must be worn during all activities

7. Photo/Video Release

I grant NXTGENFUTBOL permission to use photographs and video recordings of my child taken during the Activities for promotional purposes, including but not limited to social media, website, and marketing materials.

8. Acknowledgment

I have carefully read this Liability Waiver and Release Form and fully understand its contents. I am aware that this is a release of liability and a contract between myself and NXTGENFUTBOL and I sign it of my own free will.
